What to expect

All visits will be at client’s home. Visits will range from 50-60 minutes depending on type of visit and needs. Follow-up visits anticipated to be every 2 months.

Services provided are either Routine or Conservative Nail Care (the latter for those with diabetes, neuropathy, etc). Recommendations for follow-up with medical providers will be advised as needed. As a Clinical Nurse Leader, evidence-based practice is and will be my guiding star.

Initial visit to include obtaining contact details and preferences, history, and consents, with setting up of health record. Feet will be assessed first, and referrals made, for concerning issues prior to performing any services. Explanation/education is typically shared throughout visit.

If deemed appropriate to continue, feet will be cleaned, toenails sanded as needed with vacuum sander, and debrided to length appropriate based on patient's history and preferences with an eye to promote their growing out smoothly. Feet will again be cleaned afterward and lotion applied.

Calluses and corns to be tended conservatively as part of Routine Foot Care.

Shoes will be examined for wear, if requested, with recommendations and appropriate education to be provided on footwear.

Tools are single-patient use and appropriate disinfection of all tools is done prior to reuse. Standard protocols for PPE to be used by provider. Masks are offered, and strongly recommended for patients, if sanding is to be done, especially if one has a history of respiratory issues.

Foot Care Nursing

"... older adults may experience difficulty caring for their feet as a result of physical or mental decline, decreased flexibility, body habitus, poor vision, and arthritic conditions. Older adults also experience more comorbid conditions. For example, diabetes and peripheral arterial disease are the main causes of nontraumatic lower extremity amputation. Substandard foot care typically precedes the domino effect of ulceration, infection, amputation, and cardiovascular decline due to decreased activity. ... Access to quality foot care can prevent complications and major lower-limb amputations."
